Here's the thing to keep in mind with Actifed DM: the pseudoephedrine in it is a decongestant that revs up your nervous system, much like caffeine does. Yerba mate brings its own dose of caffeine to the party. Stack the two together and you may feel the stimulant effects pile on, things like a racing or pounding heart, jitteriness, trouble sleeping, anxiety, or a bump in blood pressure.

This is more of a theoretical, common-sense caution based on expert opinion than a proven, dangerous reaction. A single cup of mate alongside this medicine probably won't cause trouble for most folks, but heavy mate or several energy drinks could tip you over. If you have high blood pressure or a heart condition, check with your pharmacist or doctor first.

Theoretically, yerba mate might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.

Here's the thing to keep in perspective: this combo product (Actifed DM) contains dextromethorphan for cough, pseudoephedrine as a decongestant, and triprolidine as an antihistamine. Lab studies suggest yerba mate may mildly slow down a liver enzyme called CYP3A4, which helps clear dextromethorphan. In theory, that could let a little more dextromethorphan build up, possibly nudging up side effects like drowsiness, dizziness, or feeling foggy.

But honestly, this is rated minor and is based only on test-tube data, not real people. What I'd watch for more is the caffeine in yerba mate stacking with pseudoephedrine, since both are stimulants and together they can leave you jittery, restless, or with a racing heart. If you feel wired, ease up on the mate and check with your pharmacist.
