Doravirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Yerba Mate interacts with.

Theoretically, yerba mate might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.

Here's the gist for your HIV medication doravirine (Pifeltro). Your body relies on a liver enzyme called CYP3A4 to clear this drug. In lab dish studies, yerba mate seemed to slow that enzyme down a bit. If that happened in real people, doravirine could build up slightly, which in theory might mean more of its usual side effects, like headache, nausea, dizziness, or trouble sleeping.

I want to keep this in perspective, though. This is rated a minor interaction based only on test-tube research, not on actual patients, so the real-world risk looks low. Doravirine is not a prodrug, so there's no concern about it working less. Still, since steady HIV treatment matters, it's worth a quick mention to your pharmacist or doctor if you drink mate regularly.

What Is Doravirine Used For?

Brand name: Pifeltro

Pifeltro (doravirine) is used together with other antiretroviral medications to treat HIV-1 infection in adults and in pediatric patients weighing at least 35 kg (about 77 lbs). It is approved for people who have never been on HIV treatment before, as well as for people who are already virologically suppressed — meaning their HIV is currently undetectable — and want to switch to a new stable regimen.

Pifeltro is not used alone; it must always be part of a combination HIV regimen. Patients switching to a Pifeltro-based regimen must have no history of treatment failure and no known resistance to doravirine.
