Theoretically, the caffeine in yerba mate may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Yerba mate contains caffeine. Caffeine is reported to have antiplatelet activity. Theoretically, it might increase the risk of bleeding when used concomitantly with these agents; however, this interaction has not been reported in humans.

Here's the situation with your IV treprostinil (Remodulin). This medicine doesn't just open up blood vessels in your lungs, it also makes your platelets a little less sticky, so it can mildly raise bleeding risk on its own. The concern with yerba mate is that the caffeine in it has shown some platelet-slowing activity in lab studies, which could in theory add to that effect and make easy bruising or minor bleeding a bit more likely.

I want to be honest about the strength of this: it's theoretical, based only on test-tube and animal data, and it has never actually been reported in people drinking mate. A casual cup now and then is very unlikely to cause trouble. Still, since treprostinil is a serious infused medicine, mention your mate habit to your pharmacist or care team and watch for any unusual bruising or bleeding.

About Treprostinil Injection

What Is Treprostinil Injection Used For?

Brand name: Remodulin

Treprostinil injection is used to treat pulmonary arterial hypertension (PAH, WHO Group 1) — high blood pressure in the arteries that supply the lungs. It works to reduce the symptoms that make exercise difficult. Clinical studies included patients with moderate-to-severe PAH symptoms (NYHA Class II–IV) due to idiopathic or heritable PAH, PAH related to congenital heart defects, and PAH related to connective tissue diseases.

It is also used for PAH patients who need to transition off epoprostenol — a related medication — to slow the worsening of their condition during that switch. The risks and benefits of the transition should be carefully discussed with a specialist before starting.
