Theoretically, bacopa might increase the levels and adverse effects of CYP1A2 substrates.
Research on the effects of bacopa extracts on CYP1A2 enzymes is conflicting. Some in vitro evidence shows that bacopa extract can moderately and non-competitively inhibit CYP1A2, while other in vitro evidence suggests that any effect is unlikely to be clinically significant.

Here's the gist for ondansetron (Zofran). Lab studies hint that bacopa might slow down a liver enzyme called CYP1A2, which is one of several pathways your body uses to clear ondansetron. If that enzyme is blocked, in theory ondansetron could hang around a little longer and its effects (and side effects) could be slightly stronger. With ondansetron, that might mean more headache, constipation, or tiredness, and rarely heart-rhythm changes at higher levels.

The reassuring part: this is only test-tube evidence, and even those studies disagree on whether it matters in real people. Ondansetron also leans more on other enzymes, so a big problem is unlikely. Still, if you take both regularly, it's worth a quick mention to your pharmacist or doctor.

Your body relies on a liver enzyme called CYP3A4 to help clear ondansetron. In test tube studies, bacopa has shown a possible ability to slow that same enzyme down. If that were to happen in real life, ondansetron could hang around a bit longer than usual, which in theory might bump up its effects and side effects like headache, constipation, dizziness, or tiredness.

Here's the honest part: this concern comes only from lab research, and even those studies disagree with each other. Some suggest any effect would be too small to notice. So I wouldn't lose sleep over it, but it's worth a quick mention to your pharmacist or doctor, especially if you notice new headaches or constipation while using both.

Bacopa is an Ayurvedic herb most often used for memory and thinking. Some small studies suggest it may modestly help memory when taken regularly for several weeks, but the evidence is limited and quality varies. It is generally well tolerated but can cause stomach upset, and it should be avoided in pregnancy and breastfeeding due to a lack of safety data.

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
