Acetaminophen, Aspirin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know about Gemnisyn: it pairs acetaminophen with aspirin, and aspirin already works by keeping your platelets from sticking together and forming clots. That's part of how it eases pain and inflammation, but it also makes you bleed a little more easily. Chanca piedra contains a plant compound that, in lab dish studies, seemed to slow platelets down too. So in theory, taking both could nudge your bleeding risk up a bit.
I want to be honest about how soft this evidence is. It comes from test-tube research only and has never been shown in actual people. Still, since aspirin is already in the mix, keep an eye out for easy bruising, nosebleeds, or bleeding gums, and check with your pharmacist or doctor before combining them.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the short version on combining chanca piedra with this acetaminophen and aspirin combo (Gemnisyn). In lab dish studies, chanca piedra seems to speed up a liver enzyme called CYP1A2. Since your body uses that enzyme to help process certain drugs, the theory is that the herb could clear them a bit faster, which might mean slightly weaker pain or fever relief. Neither acetaminophen nor aspirin is a prodrug, so this isn't about them becoming stronger.
Honestly, I wouldn't lose sleep over this one. It's only been seen in test tubes, never in real people, and CYP1A2 isn't even the main pathway for these two. Still, if you take this regularly, it's worth a quick word with your pharmacist or doctor.
