In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the gist for your Fioricet with Codeine. The piece that matters here is the caffeine, which your liver clears using an enzyme called CYP1A2. Lab studies (in test tubes, not people) hint that chanca piedra may speed that enzyme up. In theory, that could clear caffeine a bit faster, so you might notice less of the caffeine pep, or your usual coffee feeling weaker.

Honestly, this is a theoretical concern only, never actually reported in humans, so I wouldn't lose sleep over it. The codeine, acetaminophen, and butalbital parts aren't really driven by this enzyme. Still, if you're using chanca piedra regularly, just give your pharmacist or doctor a heads-up so they can keep an eye on things.

This combo pill is a mix, and the part that matters most here is butalbital, the barbiturate that helps with tension headaches. Your liver uses an enzyme called CYP3A4 to help clear butalbital (and to a lesser degree the caffeine). Lab studies suggest chanca piedra can slow that enzyme down a bit. In theory, that could let these ingredients linger longer and feel stronger, meaning more drowsiness, dizziness, lightheadedness, or that foggy, sluggish feeling.

I'll be honest with you though: this is only seen in test-tube research and has never been reported in real people, so it's more of a caution than a proven problem. Still, since this medicine already causes sedation, it's worth a quick chat with your pharmacist or doctor before combining them.
