Acetaminophen, Caffeine, Codeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

This combo pill mixes three things, and the part that lines up with chanca piedra is the caffeine. Caffeine is handled by a liver enzyme called CYP1A2. In test-tube studies, chanca piedra seems to speed that enzyme up, which could clear caffeine a bit faster. In plain terms, the caffeine might feel a little weaker or wear off sooner, so you may notice less of the alertness or pick-me-up it usually gives.

Keep in mind this is only seen in lab research, not in real people, so it's theoretical and likely minor. The codeine and acetaminophen pieces aren't the concern here. Before mixing the two regularly, just run it by your pharmacist or doctor to be safe.

In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

This combination pill mixes acetaminophen, caffeine, and a little codeine. The codeine part is worth a closer look. Lab studies suggest chanca piedra can slow down a liver enzyme called CYP3A4, and the caffeine in this product is partly handled by that same pathway. If it slows clearance, caffeine could linger a bit longer, which might mean more jitteriness, a racing heart, or trouble sleeping.

Here's the honest part: this is only a test-tube finding, and it has never actually been reported in real people. So I wouldn't lose sleep over it, but I also wouldn't ignore it. Mention the chanca piedra to your pharmacist or doctor, especially since codeine and acetaminophen both deserve careful dosing anyway.
