Alogliptin, Pioglitazone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Oseni combines two diabetes medicines, alogliptin and pioglitazone. The piece that matters here is pioglitazone, which your liver clears partly through an enzyme called CYP3A4. In lab studies (not in actual people), chanca piedra appears to slow that enzyme down. In theory, that could let pioglitazone build up a bit and push its effects, like extra fluid retention, mild swelling, weight gain, or low blood sugar if you also take other glucose-lowering drugs.

Keep in mind this is only a test-tube finding and has never been reported in real patients, so I wouldn't lose sleep over it. Still, since you're managing blood sugar, it's worth a quick chat with your pharmacist or doctor before adding chanca piedra so they can watch how you're doing.

Theoretically, concomitant use with antidiabetes drugs might affect glucose control and increase the risk of hypoglycemia.
Animal research suggests that chanca piedra can have hypoglycemic effects. However, a small clinical study in adults with diabetes shows that chanca piedra extract 25 grams orally daily for 1 week does not lower fasting or postprandial blood glucose levels.

Here's the simple version. Oseni combines two diabetes medicines, alogliptin and pioglitazone, that help lower your blood sugar. Chanca piedra has shown a mild blood-sugar-lowering effect in animal studies, so the theory is that stacking the two could push your glucose a little too low. In practice, though, a small human study found that even 25 grams of chanca piedra extract daily for a week did not change fasting or after-meal blood sugar.

So the real-world risk here is low, and the evidence is mostly anecdotal. Still, if you do try chanca piedra, it's smart to watch for signs of low blood sugar like shakiness, sweating, or feeling lightheaded, and check your levels as usual.
