Apixaban falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.

Here's the short version: Eliquis (apixaban) is already a blood thinner, and there's a theoretical concern that chanca piedra could nudge your bleeding risk a little higher if you take them together. In lab studies, one compound found in this herb seemed to make platelets (the cells that help blood clot) a bit less sticky. That same effect has not actually been seen in people, so this is more of a 'on paper' caution than a proven problem.

Still, since apixaban itself raises bleeding risk, I'd keep an eye out for things like easy bruising, nosebleeds, bleeding gums, or unusually heavy or long-lasting bleeding. Before adding chanca piedra, please run it by your pharmacist or doctor so they can factor in your full situation.

Apixaban also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the connection: your body partly clears Eliquis (apixaban) using a liver enzyme called CYP3A4. In lab tests (test tube, not people), chanca piedra appears to slow that same enzyme down. If that held true in the body, your apixaban could clear a little slower and build up, which with a blood thinner means a higher chance of bruising or bleeding (nosebleeds, bleeding gums, or more serious bleeding).

I want to be honest about the strength of this, though. It's theoretical and has not been seen in actual patients. Still, since apixaban is a drug where even small shifts matter, I'd loop in your pharmacist or doctor before combining them rather than starting or stopping anything on your own.

About Apixaban

What Is Apixaban Used For?

Brand name: Eliquis

Apixaban is used to reduce the risk of stroke and dangerous clots traveling through the body in adults with nonvalvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). It is also used to prevent deep vein thrombosis (DVT — a blood clot in the leg) in adults after hip or knee replacement surgery, and to treat DVT and pulmonary embolism (PE — a blood clot in the lungs) in adults, including reducing the risk of those clots coming back.

Eliquis (the brand-name tablet and kit) is additionally approved to treat venous thromboembolism (VTE — blood clots in the veins) and reduce the risk of recurrent VTE in pediatric patients from birth and older, after at least 5 days of initial anticoagulant treatment. The generic apixaban tablet carries the adult indications only.
