Chlorothiazide, Reserpine falls under DIURETIC DRUGS — the group Chanca Piedra interacts with.

Theoretically, concomitant use of chanca piedra with diuretics might increase diuresis.
Some preliminary clinical research in adults with hypertension shows that chanca piedra has diuretic properties. However, higher quality research in adults with kidney stones shows taking chanca piedra does not increase urine volume when compared with placebo. Until more is known, use cautiously in patients taking diuretic drugs.

Diupres pairs two ingredients: chlorothiazide, a water pill, and reserpine, which lowers blood pressure. The concern here is pretty straightforward. Chanca piedra has been used traditionally as a 'stone breaker,' and a little early research hints it may have a mild diuretic effect of its own. Stack that on top of a medicine that already makes you urinate more, and in theory you could lose a bit more fluid than expected.

If that happens, you might notice things like lightheadedness, dizziness when standing, extra thirst, or feeling drained. Keep in mind this is theoretical and based on pharmacology, not strong studies (one good trial actually showed no increase in urine output). Still, I'd run it past your pharmacist or doctor before combining them, just to be safe.

Theoretically, concomitant use of chanca piedra with antihypertensive drugs might have additive blood pressure lowering effects.
Animal research suggests that chanca piedra can decrease blood pressure. However, this effect was not observed in most hypertensive patients treated with chanca piedra for 10 days.

Your Diupres combines two blood pressure medicines: chlorothiazide, a water pill, and reserpine, which calms the nervous system's effect on your vessels. Chanca piedra has been studied as a mild blood pressure lowerer, so the concern here is simple: stacking it on top of medicine that already lowers your pressure could, in theory, push it down a little too far.

If that happened, you might feel lightheaded, dizzy when standing, or unusually tired. Honestly though, this is a theoretical worry based mostly on animal studies, and human research didn't show much drop over 10 days. So it's unlikely to cause trouble for most people. Still, if you check your pressure at home, keep an eye on it, and chat with your pharmacist or doctor before adding this herb.
