Defibrotide Sodium falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.

Here is the gist: defibrotide (Defitelio) is a clot-fighting medicine given in the hospital to people with serious liver blood-vessel problems after a stem-cell transplant. It already lowers your blood's ability to clot. The concern with chanca piedra is that, in lab studies, one of its plant compounds slowed platelets (the cells that help form clots) from sticking together. Stacking two things that both discourage clotting could, in theory, raise your bleeding risk, think easy bruising, nosebleeds, bleeding gums, or harder-to-stop bleeding.

That said, this is only test-tube science and has never been shown in real people, so I don't want to overstate it. Honestly, defibrotide is used in a closely monitored hospital setting, so adding any supplement should always be cleared with your care team first.

What Is Defibrotide Injection Used For?

Brand name: Defitelio

Defitelio (defibrotide injection) is used to treat hepatic veno-occlusive disease (VOD) — also called sinusoidal obstruction syndrome (SOS) — in adult and pediatric patients. This is a serious condition where small blood vessels in the liver become blocked, most often after a hematopoietic stem-cell transplant (HSCT).

Defitelio is specifically indicated when VOD is accompanied by kidney dysfunction or pulmonary (lung) dysfunction, making it a treatment for one of the most severe forms of this complication.
