Ibrutinib falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.

Here's the connection worth knowing about. Ibrutinib (Imbruvica) already carries its own bleeding risk because of how it works on your platelets, the tiny cells that help your blood clot. Chanca piedra contains a plant compound that, in lab dish studies, seemed to slow platelets down too. Stack the two together and the concern is that bleeding could happen more easily than expected.

I want to be honest about the strength of this, though. This is theoretical, based only on test-tube research, and it has never actually been reported in people. So please don't panic. Still, with a drug like ibrutinib, watch for easy bruising, nosebleeds, bleeding gums, or unusually dark stools, and let your oncologist or pharmacist know before adding chanca piedra so they can weigh in.

Ibrutinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the situation with ibrutinib (Imbruvica). Your body clears this cancer medicine using a liver enzyme called CYP3A4. Lab studies suggest chanca piedra may slow that same enzyme down. If that happened in real life, ibrutinib could build up in your blood, which can mean more side effects like bruising or bleeding, diarrhea, low blood counts, infections, or heart rhythm changes.

I want to be honest about the strength of this, though. So far this is only seen in test-tube research, not in actual people, so it's more of a caution than a proven problem. Ibrutinib is a serious drug with real interactions, so please don't add chanca piedra without first checking with your oncologist or pharmacist. They can tell you if it's safe for your specific plan.

About Ibrutinib

What Is Ibrutinib Used For?

Brand name: Imbruvica

Imbruvica (ibrutinib) is used in adults to treat chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL), including cases with a specific genetic change called 17p deletion. It is also used in adults to treat Waldenström's macroglobulinemia (WM), a rare type of slow-growing blood cancer.

Imbruvica is also approved for adults and children aged 1 year and older with chronic graft-versus-host disease (cGVHD) — a serious immune complication that can occur after a stem cell or bone marrow transplant — when at least one prior treatment has not worked.
