Ibuprofen, Acetaminophen falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Advil Dual Action, which combines ibuprofen (an NSAID) and acetaminophen. Ibuprofen on its own can already nudge up bleeding risk because it affects how your platelets clump together. In lab studies, one compound found in chanca piedra also slowed platelet clumping. Put those two together, in theory, and you might see a little more tendency toward bleeding or bruising.
I want to be honest about how soft this is: that platelet effect has only been seen in a test tube, never proven in people. So for most folks it's a low-key concern. Still, if you notice easy bruising, nosebleeds, or stomach upset, or if you take blood thinners, check with your pharmacist or doctor first.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
2 of 2Ibuprofen, Acetaminophen also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the gist with Advil Dual Action (ibuprofen plus acetaminophen). In lab dish studies, chanca piedra seems to speed up a liver enzyme called CYP1A2. If that happened in the body, it could clear these pain relievers a little faster, which in theory might make them work less well, so you'd get less relief for that headache, muscle ache, or back pain.
I want to be honest about how thin this evidence is. It comes only from test-tube research, and nothing like it has actually been reported in real people, so for most folks I wouldn't lose sleep over it. Still, if you take chanca piedra regularly and notice your pain isn't easing like it used to, mention it to your pharmacist or doctor.
