Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the situation with Kalydeco (ivacaftor). Your body relies on an enzyme called CYP3A4 to clear this drug out. In lab studies (test tube only, not in people), chanca piedra appears to slow that enzyme down. If that held true in the body, ivacaftor could build up a bit higher than expected, which might mean more of its usual side effects like headache, dizziness, belly pain, diarrhea, or a rash.

I want to be honest about how thin this evidence is. It's theoretical and has never actually been seen in a real patient, so I'm not ringing alarm bells. Still, since ivacaftor is a long-term, carefully dosed CF medicine, it's worth a quick chat with your pharmacist or CF team before adding chanca piedra. Don't change your dose on your own.

About Ivacaftor

What Is Ivacaftor Used For?

Brand name: Kalydeco

Kalydeco (ivacaftor) is used to treat cystic fibrosis (CF) in patients aged 1 month and older who have at least one mutation in the CFTR gene that responds to ivacaftor. Because only certain mutations respond to this drug, a genetic test is used to confirm eligibility before treatment begins.

A large number of specific CFTR mutations are recognized as responsive, including G551D, R117H, F508C, S549N, S549R, and many others listed in the FDA-approved prescribing information. If a patient's mutation is unknown, an FDA-cleared CF mutation test should be used to identify it.
