Mannitol falls under DIURETIC DRUGS — the group Chanca Piedra interacts with.

Theoretically, concomitant use of chanca piedra with diuretics might increase diuresis.
Some preliminary clinical research in adults with hypertension shows that chanca piedra has diuretic properties. However, higher quality research in adults with kidney stones shows taking chanca piedra does not increase urine volume when compared with placebo. Until more is known, use cautiously in patients taking diuretic drugs.

Here's the gist: chanca piedra has been studied as a mild 'water pill' itself, and mannitol (Osmitrol) is a powerful osmotic diuretic that pulls extra fluid into your urine. The concern on file is purely theoretical, meaning that on paper, stacking two things that both increase urination could add up. In practice, this is weak: the better research in people with kidney stones actually found chanca piedra did not increase urine volume versus placebo.

Mannitol is given in a hospital setting (often for brain or eye pressure, or to jump-start urine output), so your team is already watching your fluids, electrolytes, and blood pressure closely. If extra fluid loss did occur, you might feel dizzy, lightheaded, or dehydrated. Please don't start or stop anything on your own. Just mention the chanca piedra to your doctor or pharmacist so they can keep an eye on things.

Animal research suggests that chanca piedra can decrease blood pressure. However, this effect was not observed in most hypertensive patients treated with chanca piedra for 10 days.

Here's the short version for this pairing. Chanca piedra has shown a mild blood pressure lowering effect in some animal studies, and your interaction file groups it with blood pressure medicines. The thinking is that adding chanca piedra could nudge your pressure down a little further.

That said, mannitol isn't a typical blood pressure pill. It's an osmotic diuretic given in a hospital setting (Osmitrol) to pull fluid out, lower pressure in the brain or eyes, or boost urine output, and Bronchitol is an inhaled form for cystic fibrosis. Any overlap here is purely theoretical, and one human study didn't even see a blood pressure drop. Realistically, your care team is watching your fluids and pressure closely anyway. Still, mention any supplements to your doctor or pharmacist so they can keep an eye out.
