Perampanel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the situation with Fycompa (perampanel) and chanca piedra. Your body uses a liver enzyme called CYP1A2 to help break down perampanel. In test tube studies, chanca piedra seems to speed up that enzyme. If that holds true in people, it could clear perampanel out faster and lower its levels in your blood.

For a seizure medicine, the worry isn't extra side effects, it's the opposite: less drug could mean your seizures aren't controlled as well as before. That said, this is only based on lab research and has never actually been reported in a real person, so it's more of a theoretical caution than a proven problem. Please loop in your pharmacist or neurologist before adding chanca piedra so they can keep an eye on your seizure control.

In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down perampanel (Fycompa) and clear it out. In lab dish studies, chanca piedra appears to slow that enzyme down. If that same effect happened in people, perampanel could clear more slowly and build up a bit, which might nudge up its usual side effects like dizziness, drowsiness, trouble with balance or coordination, irritability, or mood changes.

I want to be straight with you, though: this is only a theoretical concern based on test-tube research, and it has never actually been reported in humans. So I'm not raising an alarm. Still, since perampanel already causes sleepiness and dizziness for many folks, it's worth a quick chat with your pharmacist or doctor before combining the two.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
