These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Chanca Piedra interacts with whole categories of medication. Polythiazide, Reserpine falls into 2 of those categories, so it can interact with Chanca Piedra in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
DIURETIC DRUGS
1 of 2Why this applies to Polythiazide, Reserpine
Polythiazide, Reserpine falls under DIURETIC DRUGS — the group Chanca Piedra interacts with.
Some preliminary clinical research in adults with hypertension shows that chanca piedra has diuretic properties. However, higher quality research in adults with kidney stones shows taking chanca piedra does not increase urine volume when compared with placebo. Until more is known, use cautiously in patients taking diuretic drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Renese-R combines two blood pressure medicines: polythiazide, a water pill (diuretic), and reserpine. Here's the idea behind this interaction. Chanca piedra has been studied for possibly having a mild diuretic effect of its own, so the worry is that pairing it with your thiazide could add up and make you lose a bit more fluid than expected.
If that happened, you might feel lightheaded or dizzy (especially standing up), get dehydrated, or run low on potassium, which can cause muscle cramps or a fast heartbeat. Honestly though, this is theoretical, and the better-quality study actually didn't show more urine output. So the risk is likely small. Still, check with your pharmacist or doctor before adding it, and don't change your Renese-R on your own.

ANTIHYPERTENSIVE DRUGS
2 of 2Polythiazide, Reserpine also falls under ANTIHYPERTENSIVE DRUGS.
Animal research suggests that chanca piedra can decrease blood pressure. However, this effect was not observed in most hypertensive patients treated with chanca piedra for 10 days.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Renese R combines two blood pressure medicines: polythiazide (a water pill) and reserpine (which calms the nervous system's grip on your blood vessels). Both work to lower your blood pressure. The thought here is that chanca piedra might nudge your blood pressure down a little more on its own, so stacking it on top could add to that effect.
Honestly, this is a very theoretical concern. It comes from animal studies, and when actual people with high blood pressure took chanca piedra for 10 days, most didn't see a drop. Still, if your pressure ran too low you could feel dizzy, lightheaded, or tired, especially when standing up. If you'd like to try this herb, just check in with your pharmacist or doctor first so they can keep an eye on your numbers.

From our pharmacist’s overview on the Polythiazide, Reserpine page
“American hellebore is the ingredient I would keep farthest from polythiazide with reserpine (Renese R), a combination pill prescribed to lower blood pressure. Its alkaloids can drop pressure and slow the heart even at small doses, and it is one of ten major-rated entries among the 228 on file here, which puts this list in the take-it-s…”

- Kidney stones
- Gallstones
- Liver support
- Urinary tract health
- Digestive complaints
Chanca piedra is a tropical herb traditionally used as a 'stone breaker' for kidney and gallstones, and for liver and urinary health.
