Rifabutin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the gist for Rifabutin. Your liver uses an enzyme called CYP3A4 to clear this drug from your body. In lab (test tube) studies, chanca piedra appears to slow that enzyme down. In theory, that could let rifabutin build up a bit higher than usual, which might mean more of its typical side effects, like nausea, an upset stomach, a rash, or changes in your blood counts. Rifabutin can also turn your urine, tears, and skin a harmless orange-brown color.

The important part: this is only a theoretical concern from laboratory work, and it has never actually been reported in people. So I wouldn't panic. Still, since rifabutin is a serious antibiotic, please run this combination by your pharmacist or doctor before mixing the two.

Brand name: Mycobutin

Rifabutin capsules are used to prevent disseminated MAC (Mycobacterium avium complex) disease — a serious bacterial infection that can spread throughout the body — in patients with advanced HIV infection.

It is not approved to treat active tuberculosis (TB), and it must not be used alone in patients who have active TB, as this can lead to antibiotic resistance.
