Riociguat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the situation with these two together. Your body relies on a liver enzyme called CYP3A4 to help clear riociguat (Adempas). In lab dish studies, chanca piedra appears to slow that same enzyme down. If that were to happen in real life, less of the drug would get broken down, so more could build up in your bloodstream.

Why does that matter? Riociguat already relaxes and widens blood vessels, so higher levels could push your blood pressure too low. That can leave you dizzy, lightheaded, or faint, especially when you stand up. I want to be honest, though: this is only a theoretical concern from lab work, and it has not been reported in actual people. Still, because riociguat is a sensitive medication, please run it by your pharmacist or doctor before combining them.

Theoretically, concomitant use of chanca piedra with antihypertensive drugs might have additive blood pressure lowering effects.
Animal research suggests that chanca piedra can decrease blood pressure. However, this effect was not observed in most hypertensive patients treated with chanca piedra for 10 days.

Here's the simple version: Riociguat (Adempas) already lowers blood pressure because it relaxes blood vessels, including those in your lungs. Chanca piedra has shown a mild blood-pressure-lowering effect in animal studies, so in theory stacking the two could nudge your pressure down a little more. If that happened, you might feel dizzy, lightheaded, or faint, especially when standing up quickly, and you could notice headaches or an upset stomach.

That said, this is a theoretical concern only. In a small human study, chanca piedra didn't actually drop blood pressure in most people over 10 days. So the real-world risk looks low. Still, since riociguat is closely tied to blood pressure, it's worth a quick chat with your pharmacist or doctor before adding chanca piedra, and don't change your medication on your own.

What Is Riociguat Used For?

Brand name: Adempas

Riociguat — sold as Adempas and under the generic name Riociguat — is used to treat adults with two conditions that cause dangerously high blood pressure in the arteries of the lungs.

  • Chronic Thromboembolic Pulmonary Hypertension (CTEPH): Used after surgical treatment, or when surgery is not an option, to improve exercise capacity and functional class.
  • Pulmonary Arterial Hypertension (PAH): Used to improve exercise capacity, functional class, and to delay the condition from getting worse. It can be used alone or combined with certain other PAH medicines.
