Spironolactone falls under DIURETIC DRUGS — the group Chanca Piedra interacts with.

Theoretically, concomitant use of chanca piedra with diuretics might increase diuresis.
Some preliminary clinical research in adults with hypertension shows that chanca piedra has diuretic properties. However, higher quality research in adults with kidney stones shows taking chanca piedra does not increase urine volume when compared with placebo. Until more is known, use cautiously in patients taking diuretic drugs.

Here's the gist: chanca piedra has been suggested to act a bit like a water pill itself, though the evidence is shaky. Some early blood pressure studies hint at a mild diuretic effect, while a better study in folks with kidney stones found no real change in urine output. So this is more of a theoretical concern than a proven problem.

If both nudge your body to release extra fluid, you could in theory notice more urination, lightheadedness, or a dip in blood pressure when standing. Since spironolactone is a potassium-sparing diuretic, the bigger thing I'd watch is staying hydrated and not overdoing fluid loss. This isn't a dangerous combo for most people, but please run it by your pharmacist or doctor before adding the herb, especially if you check potassium or kidney labs.

Theoretically, concomitant use of chanca piedra with antihypertensive drugs might have additive blood pressure lowering effects.
Animal research suggests that chanca piedra can decrease blood pressure. However, this effect was not observed in most hypertensive patients treated with chanca piedra for 10 days.

Here's the gist: chanca piedra may have a mild blood pressure lowering effect of its own, and spironolactone is often used to bring blood pressure down too. On paper, stacking the two could nudge your blood pressure a little lower than expected. That might show up as feeling lightheaded, dizzy, or woozy when you stand up quickly.

That said, I don't want to alarm you. This concern comes mostly from animal studies, and in a human study chanca piedra didn't lower blood pressure in most people over ten days. So it's a theoretical, low-likelihood interaction. If you'd like to try it, just check with your pharmacist or doctor first, and watch how you feel, especially the first week.

About Spironolactone

What Is Spironolactone Used For?

Brand names: Aldactone, Spiroton, Carospir

Spironolactone tablets (including Aldactone) are used to treat moderate-to-severe heart failure, high blood pressure, and fluid retention (edema) caused by liver cirrhosis or nephrotic syndrome. They are also used to diagnose and manage primary hyperaldosteronism — a condition where the adrenal glands produce too much of the hormone aldosterone.

Spironolactone oral suspension shares several of these uses — heart failure, high blood pressure, and edema from liver cirrhosis — but is not the same as Aldactone and is not therapeutically interchangeable with it. Your prescriber will choose the right form for your situation.
