Chanca Piedra and Tolazamide

Here is the interaction we found between these two, with the evidence behind it.

Minor

ANTIDIABETES DRUGS

Why this applies to Tolazamide

Tolazamide falls under ANTIDIABETES DRUGS — the group Chanca Piedra interacts with.

Theoretically, concomitant use with antidiabetes drugs might affect glucose control and increase the risk of hypoglycemia.
Animal research suggests that chanca piedra can have hypoglycemic effects. However, a small clinical study in adults with diabetes shows that chanca piedra extract 25 grams orally daily for 1 week does not lower fasting or postprandial blood glucose levels.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: tolazamide already lowers your blood sugar by nudging your pancreas to release more insulin. The thinking is that chanca piedra might have its own mild blood-sugar-lowering effect, so stacking the two could, in theory, push your sugar a little too low. With tolazamide, low blood sugar can show up as shakiness, sweating, dizziness, hunger, or feeling confused.

That said, I don't want to overstate this. The worry mostly comes from animal studies, and a small human study found chanca piedra didn't actually change blood sugar in people with diabetes. So this is more of a theoretical 'keep an eye out' than a real red flag. If you'd like to try it, that's a great conversation to have with your doctor or pharmacist, and checking your sugar a bit more often early on is a smart, simple safeguard.

From our pharmacist’s overview on the Tolazamide page

“Tolazamide (Tolinase) lowers blood sugar in type 2 diabetes, and I would handle its supplement list with care, because nearly everything listed can drag blood sugar lower still. Herbs and foods with glucose-lowering effects dominate the file, and stacked on a pill that already lowers glucose they raise the odds of hypoglycemia, blood s…”

About Chanca Piedra

What is Chanca Piedra?

Chanca Piedra
  • Kidney stones
  • Gallstones
  • Liver support
  • Urinary tract health
  • Digestive complaints

Chanca piedra is a tropical herb traditionally used as a 'stone breaker' for kidney and gallstones, and for liver and urinary health. Human evidence for these uses is limited and mostly small or preliminary, so it should not replace proven treatments. Talk with your pharmacist or doctor before using it, especially if you have a medical condition or take medications.

About Tolazamide

What Is Tolazamide Used For?

Brand name: Tolinase

  • Tolazamide is used along with diet and exercise, and sometimes with other medications, to treat type 2 diabetes (condition in which the body does not use insulin normally and, therefore, cannot control the amount of sugar in the blood). Tolazamide is in a class of medications called sulfonylureas. Tolazamide lowers blood sugar by causing the pancreas to produce insulin (a natural substance that is needed to break down sugar in the body) and helping the body use insulin efficiently. This medication will only help lower blood sugar in people whose bodies produce insulin naturally. Tolazamide is not used to treat type 1 diabetes (condition in which the body does not produce insulin and, therefore, cannot control the amount of sugar in the blood) or diabetic ketoacidosis (a serious condition that may occur if high blood sugar is not treated).

  • Over time, people who have diabetes and high blood sugar can develop serious or life-threatening complications, including heart disease, stroke, kidney problems, nerve damage, and eye problems. Taking medication(s), making lifestyle changes (e.g., diet, exercise, quitting smoking), and regularly checking your blood sugar may help to manage your diabetes and improve your health. This therapy may also decrease your chances of having a heart attack, stroke, or other diabetes-related complications such as kidney failure, nerve damage (numb, cold legs or feet; decreased sexual ability in men and women), eye problems, including changes or loss of vision, or gum disease. Your doctor and other healthcare providers will talk to you about the best way to manage your diabetes.
