Troglitazone falls under ANTIDIABETES DRUGS — the group Chanca Piedra interacts with.

Theoretically, concomitant use with antidiabetes drugs might affect glucose control and increase the risk of hypoglycemia.
Animal research suggests that chanca piedra can have hypoglycemic effects. However, a small clinical study in adults with diabetes shows that chanca piedra extract 25 grams orally daily for 1 week does not lower fasting or postprandial blood glucose levels.

Here's the gist: troglitazone (Rezulin) is a diabetes medicine that helps your body use insulin more effectively to lower blood sugar. The thought here is that chanca piedra might nudge your blood sugar down too, so stacking the two could, in theory, push you toward a low (hypoglycemia). With troglitazone alone lows are uncommon, but watch for shakiness, sweating, dizziness, or feeling foggy.

Honestly, the evidence is thin. Animal studies hinted at a blood-sugar-lowering effect, but a small human study using 25 grams daily for a week showed no real change. So this is more of a theoretical caution than a proven problem. If you do combine them, keep an eye on your glucose readings and check in with your pharmacist or doctor first.
