Warfarin Sodium falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: warfarin is a powerful blood thinner with a very narrow safety window, so anything that could nudge your bleeding risk is worth a conversation. Chanca piedra is on the radar because one of its plant compounds slowed platelets from clumping together in lab dish studies. That's a theoretical concern only, and it has not actually been seen in people taking it.
Still, when you stack a possible blood-thinning herb on top of warfarin, the worry is added bleeding. Watch for things like easy bruising, nosebleeds, bleeding gums, or blood in your urine or stool. Because warfarin is so dose-sensitive, please don't add chanca piedra on your own. Check with your pharmacist or doctor first, and keep up with your regular INR checks.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with warfarin and chanca piedra. In lab studies, chanca piedra appears to speed up an enzyme called CYP1A2, which is one of the pathways your body uses to break down warfarin. If that happens in real life, your body could clear warfarin a little faster, which might weaken its blood-thinning effect and could let your INR drift lower. A lower INR means a higher risk of the very clots warfarin is meant to prevent.
Now, the honest part: this is only theoretical and comes from test-tube work, not from people, so we don't actually know if it matters at normal doses. Still, warfarin is a narrow-margin drug where small shifts count, so please check with your pharmacist or doctor before starting it, and keep up with your regular INR checks.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with warfarin and chanca piedra. In lab (test tube) studies, this herb appears to slow down an enzyme called CYP3A4, which is one of the pathways your body uses to clear warfarin. If that pathway slows, warfarin could build up a little and thin your blood more than intended. With warfarin, that matters because it's a narrow-margin drug, meaning even small shifts can raise your bleeding risk (think easy bruising, nosebleeds, bleeding gums, or worse).
The honest part: this concern comes only from laboratory data and has never actually been reported in people, so it's theoretical. Still, because warfarin is closely monitored with INR checks, I'd loop in your pharmacist or doctor before adding chanca piedra so they can watch your numbers.

What Is Warfarin Used For?

Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.
In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
