Coffee has a moderate interaction with Adenosine Phosphate (prescription drug). Theoretically, coffee might decrease the vasodilatory effects of adenosine and interfere with its use prior to stress testing.

ADENOSINE (Adenocard)
Coffee contains caffeine. Caffeine is a competitive inhibitor of adenosine at the cellular level. However, caffeine does not seem to affect supplemental adenosine because high interstitial levels of adenosine overcome the antagonistic effects of caffeine. It is recommended that methylxanthines such as caffeine, as well as methylxanthine-containing products, be stopped 24 hours prior to pharmacological stress tests. However, methylxanthines appear more likely to interfere with dipyridamole (Persantine) than adenosine-induced stress testing.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: caffeine in coffee and adenosine work on the same receptors, but in opposite ways. Caffeine actually blocks those adenosine receptors, so in theory a cup of coffee could blunt some of the effects of adenosine, like the blood vessel widening doctors are looking for during a cardiac stress test. If the test relies on adenosine to do its job, caffeine could muddy the picture and give a less reliable result.
The good news is that when adenosine is given in the clinic, the doses are high enough to mostly overpower caffeine, so this matters less than it sounds. Still, to be safe, providers usually ask you to hold all caffeine (coffee, tea, soda, chocolate) for about 24 hours before a stress test. Always follow your own care team's prep instructions and check with them before any test.
