Why this applies to Isocarboxazid

Isocarboxazid falls under MONOAMINE OXIDASE INHIBITORS (MAOIs) — the group Coffee interacts with.

Theoretically, concomitant use might increase the risk of a hypertensive crisis.
Coffee contains caffeine. Caffeine has been shown to inhibit monoamine oxidase (MAO) A and B in laboratory studies. Concomitant intake of large amounts of caffeine with MAOIs might precipitate a hypertensive crisis. In a case report, a patient that consumed 10-12 cups of caffeinated coffee and took the MAOI tranylcypromine presented with severe hypertension. Hypertension was resolved after the patient switched to drinking decaffeinated coffee.

Here's the deal with your morning cup and Marplan (isocarboxazid). Isocarboxazid is a powerful MAO inhibitor, and caffeine on its own appears to nudge those same MAO enzymes in lab studies. The worry is that stacking a lot of caffeine on top of this medication could push your blood pressure up too far, what we call a hypertensive crisis. That can show up as a pounding headache, a racing heart, neck stiffness, sweating, or feeling suddenly very unwell.

I want to be honest about the strength of this one. It's mostly theoretical, built on pharmacology and a single case report involving someone drinking 10 to 12 cups a day. A normal cup or two probably won't be a problem for most people, but heavy daily coffee drinking is where caution makes sense. Please run your typical coffee habit by your pharmacist or doctor so they can guide you safely.

What Is Isocarboxazid Used For?

Brand name: Marplan

Marplan (isocarboxazid) is indicated for the treatment of depression — specifically, major depressive disorder. Because it carries serious risks, it is not a first-choice antidepressant. Marplan is generally reserved for patients who have not responded adequately to other antidepressants.

Clinical trials showed Marplan to be effective in adult outpatients with major depressive disorder, including those whose depression was accompanied by anxiety symptoms such as panic or phobias. Marplan is not approved for use in children or adolescents.
