Cola Nut has a moderate interaction with Adenosine. Theoretically, cola nut might decrease the vasodilatory effects of adenosine and interfere with its use prior to stress testing.

ADENOSINE (Adenocard)
Cola nut contains caffeine. Caffeine is a competitive inhibitor of adenosine at the cellular level. However, caffeine does not seem to affect supplemental adenosine because high interstitial levels of adenosine overcome the antagonistic effects of caffeine. It is recommended that methylxanthines and methylxanthine-containing products (including cola nut) be stopped 24 hours prior to pharmacological stress tests. However, methylxanthines appear more likely to interfere with dipyridamole (Persantine) than adenosine-induced stress testing.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Adenosine is usually given by a doctor during a heart stress test or to quickly correct a fast heart rhythm. It works by briefly relaxing blood vessels and slowing the heart. The caffeine in cola nut blocks the same cellular targets adenosine uses, so in theory it could blunt adenosine's effect and make a stress test less reliable.
In practice, the doses of adenosine used in a hospital are high enough to mostly override caffeine, so this concern is largely theoretical for adenosine itself (it matters more with a related drug called dipyridamole). Still, the standard advice is to stop caffeine and cola nut about 24 hours before any pharmacological stress test. Please confirm the exact timing with your doctor or pharmacist.
