Cola Nut and Flutamide

Here is the interaction we found between these two, with the evidence behind it.

Moderate

FLUTAMIDE (Eulexin)

Theoretically, cola nut might increase the levels and adverse effects of flutamide.
Cola nut contains caffeine. In vitro evidence suggests that caffeine can inhibit the metabolism of flutamide. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for cola nut and your flutamide. Cola nut's main active ingredient is caffeine, and in lab dish studies caffeine appeared to slow down the enzyme that clears flutamide from the body. If that happened in real life, flutamide levels could creep up a bit, which might mean more of its usual side effects, things like hot flashes, breast tenderness, diarrhea, or nausea. Flutamide can also stress the liver, so anything that raises its levels is worth respecting.

That said, I want to be honest about the evidence. This was only seen in a test tube, never confirmed in people, so it's theoretical. An occasional caffeinated drink is unlikely to be a problem, but a concentrated cola nut supplement is harder to predict. Please run it by your oncology team or pharmacist before adding it.

From our pharmacist’s overview on the Flutamide page

“A man taking flutamide (Eulexin) to block testosterone during prostate cancer treatment cannot afford big swings in the drug's level, and most of the 189 supplements on this page are capable of causing exactly that. The bulk of them work through the liver enzyme CYP3A4, which breaks flutamide down.”

Cola Nut
  • Energy and alertness
  • Mental focus
  • Physical performance
  • Weight loss support
  • Traditional digestive aid

Cola nut is a caffeine-containing seed from West Africa used mainly as a natural stimulant for energy and alertness. Most of its effects come from caffeine, and strong human evidence for specific health benefits is limited. Because of its caffeine content, people sensitive to stimulants and those who are pregnant or have heart conditions should be cautious.

About Flutamide

What Is Flutamide Used For?

Brand names: Eulexin, Euflex

Eulexin (flutamide) capsules are used to treat prostate cancer in men. They are always used together with an LHRH agonist (a hormone-lowering injection), never alone.

Eulexin is indicated for two stages of prostate cancer: locally confined Stage B2-C prostate cancer (used alongside goserelin acetate and radiation therapy, starting eight weeks before radiation begins) and Stage D2 metastatic prostate cancer (cancer that has spread beyond the prostate, taken with an LHRH agonist until the disease progresses).
