Romidepsin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the connection worth knowing about. Your body relies on a liver and gut enzyme called CYP3A4 to break down and clear romidepsin. Danshen can nudge that same enzyme, so it may shift how much romidepsin stays active in your system. The tricky part is that the direction depends on how it's taken. Studies suggest that taking danshen for a week or two actually speeds this enzyme up, which could lower romidepsin levels and make it work less well against your lymphoma. A single dose, oddly, seems to do the opposite and raise levels.

Because romidepsin is a powerful chemotherapy drug with serious side effects (low blood counts, nausea, heart rhythm changes), even modest changes matter. The evidence here is preliminary, so please don't start or stop anything. Just check with your oncology team or pharmacist first.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist: your body uses a transporter called P-glycoprotein to help pump certain drugs out of cells and clear them. Romidepsin (Istodax) is one of those drugs that relies on this pump. Danshen seems to slow that pump down a bit. In a small study, it raised levels of another P-glycoprotein drug by roughly 30 to 40 percent. So in theory, taking Danshen could let more Romidepsin build up in your system.

That matters because Romidepsin is a chemotherapy drug, and higher levels could mean more side effects like nausea, fatigue, low blood counts, or heart rhythm changes. This is a possible, moderate concern based on limited evidence, not a sure thing. Please don't start or stop anything on your own. Just loop in your oncology team or pharmacist first.

What Is Romidepsin Injection Used For?

Brand name: Istodax

Romidepsin injection is approved to treat cutaneous T-cell lymphoma (CTCL) — a cancer that starts in white blood cells and primarily affects the skin — in adult patients who have already received at least one prior systemic (whole-body) therapy.

It is given intravenously (through a vein) in a clinical or infusion-center setting. It is not approved or recommended for peripheral T-cell lymphoma.
