Why this applies to Tepotinib

Tepotinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the connection: your lung cancer pill tepotinib is cleared from the body by a liver and gut enzyme called CYP3A4. Danshen can nudge that same enzyme, so taking the two together may shift how much tepotinib stays in your system. Tepotinib is an active drug itself, not a prodrug, so the change tracks directly with its levels.

What makes this tricky is that danshen seems to work in two directions. Taken steadily for a couple of weeks, it appears to speed up CYP3A4, which could lower tepotinib levels and weaken its cancer-fighting effect. A single dose may do the opposite and push levels up, raising the risk of side effects like swelling, nausea, tiredness, or low protein. The evidence here is limited, so please don't start or stop anything. Just loop in your oncology team or pharmacist first.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the situation with Danshen and Tepmetko (tepotinib). Tepotinib is moved around your body partly by a transporter called P-glycoprotein, which acts like a pump that helps clear the drug. Danshen appears to slow that pump down. In one study with another P-glycoprotein drug, Danshen pushed its blood levels up by roughly a third. The same thing could happen with tepotinib, meaning more of it could build up than expected.

For tepotinib, higher levels can mean stronger side effects like swelling in your legs or face, nausea, diarrhea, tiredness, or changes in liver labs. The evidence here is moderate, not proven for tepotinib specifically, so don't panic. But please don't start Danshen without checking first. Run it by your oncology team or pharmacist.

Tepmetko (tepotinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — lung cancer that has spread — whose tumors carry a specific genetic alteration called a MET exon 14 skipping mutation. A test on tumor tissue or blood is required to confirm this mutation before treatment begins.

This is a targeted therapy, meaning it works against a specific molecular driver of cancer growth rather than attacking all rapidly dividing cells.
