Sirolimus (albumin bound) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the situation with Fyarro (albumin-bound sirolimus). Your body clears sirolimus using a liver and gut enzyme called CYP3A4. The interesting twist with Danshen is timing. In one small study of healthy men, taking it daily for 10 to 14 days actually sped up that enzyme, which would tend to lower sirolimus levels and could mean your cancer treatment works less well. But a single dose did the opposite and pushed levels up, which could mean more side effects like mouth sores, swelling, high blood sugar, or infection risk.

Because the direction can flip and sirolimus has a narrow safe range, this is hard to predict. Please don't start or stop Danshen on your own. Run it by your oncology team or pharmacist first.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the thing to know about Fyarro (the albumin-bound form of sirolimus): your body partly relies on a transporter called P-glycoprotein to move it out and keep its levels in check. Danshen has been shown in a small human study to slow that transporter down. When that happens, a drug like sirolimus can build up higher in your blood than expected.

With this cancer medicine, higher levels can mean more of its known side effects, things like mouth sores, low blood counts, infections, high blood sugar, or lung inflammation. Sirolimus also has a narrow safe range, so even modest changes matter. The evidence here is moderate and comes from a different drug, so it's a real concern but not a certainty. Please don't start or stop anything on your own. Talk with your oncology team or pharmacist before combining these.
