Acetaminophen, Chlorpheniramine, Dextromethorphan Hydrobromide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ephedra interacts with.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

Coricidin HBP is a combo cold product (acetaminophen for pain and fever, chlorpheniramine for runny nose and sneezing, and dextromethorphan for cough). The note on file is mostly theoretical and comes from animal lab work, which suggested ephedra may speed up a liver enzyme called CYP1A2. If that happened, it could slightly lower the levels of medicines cleared that way, possibly making them a touch less effective. Honestly, this enzyme effect isn't the main worry here.

The bigger, real-world concern is that ephedra is a strong stimulant that raises heart rate and blood pressure. Coricidin HBP is designed for people who already have high blood pressure, so pairing it with ephedra works against that. Ephedra is also banned in U.S. supplements. Please talk with your pharmacist before combining these.

This product, sold as Coricidin HBP Flu, contains acetaminophen (the same active ingredient as Tylenol), which your liver has to process. Ephedra-containing supplements have been linked to numerous reports of liver injury on their own. The concern here is mostly a theoretical, add-up effect: putting two things that can stress the liver together might raise the risk of liver trouble more than either alone.

To be honest, this connection is based on how these substances behave (lab and case reports), not strong human studies, so I don't want to alarm you. The bigger issue is that ephedra is banned in U.S. supplements and can dangerously raise blood pressure and heart rate. Please don't use it, and check with your pharmacist or doctor about safer options.
