Acetaminophen, Diphenhydramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ephedra interacts with.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

Here's the honest picture with Tylenol PM (acetaminophen plus diphenhydramine). The concern on file is that ephedra may speed up a liver enzyme called CYP1A2, which could clear certain medicines a little faster. But this is only based on lab and animal studies, so it's theoretical and not well proven in people. Neither acetaminophen nor diphenhydramine relies heavily on CYP1A2, so this particular enzyme effect likely won't change how well your Tylenol PM works.

The bigger real-world issue is mixing a strong stimulant with a sleep aid. Ephedra revs up your heart and blood pressure, while diphenhydramine makes you drowsy, so they pull in opposite directions and can stress your heart. Ephedra is also banned in U.S. supplements for safety reasons. Please talk with your pharmacist or doctor about safer choices.

Here's the concern with Tylenol PM. The acetaminophen part can stress your liver, especially at higher doses or with frequent use. Ephedra has been tied to a number of reported cases of liver injury on its own. So the worry is an additive one: two things that can each be hard on the liver, stacked together. This is a theoretical concern based on how each affects the body, not something proven to happen with this exact combo, so I don't want to alarm you.

One more note: ephedra is a strong stimulant and is actually banned in U.S. supplements because of serious heart and stroke risks. That's a bigger reason to steer clear. Please check with your pharmacist or doctor before using it with anything.
