Acetaminophen, Meperidine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ephedra interacts with.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

First things first: ephedra is banned from supplements in the U.S. for good reason, since it can spike your blood pressure and heart rate and has been tied to strokes and worse. So my real advice is to steer clear of it altogether, especially when you're taking Demerol APAP (meperidine plus acetaminophen), a strong opioid pain reliever.

The specific interaction on file is mostly theoretical. Some animal studies hint that ephedra may rev up a liver enzyme (CYP1A2), which could clear certain drugs a little faster and slightly weaken them. For this combo, that effect is unproven and probably minor compared to the bigger worry: ephedra's stimulant kick fighting against meperidine's sedating, blood-pressure-lowering effects. Please check with your pharmacist or doctor before mixing these.

The medicine you're asking about combines acetaminophen (the active ingredient in Tylenol) with meperidine (Demerol), a strong opioid painkiller. The concern here is mostly about your liver. Acetaminophen is one of the most common causes of liver injury when too much builds up, and there have been many reports of ephedra-containing products also harming the liver. So in theory, taking both together could add stress on top of stress.

I want to be honest: this is a theoretical concern based on how each one behaves, not proven head-to-head data. Still, ephedra is banned in U.S. supplements for good reason, and stacking two things that can hurt the liver isn't worth the risk. Please talk with your pharmacist or doctor before combining them, and ask about safer options.
