Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

Here's the honest picture with Apadaz (benzhydrocodone and acetaminophen) and ephedra. The interaction on file is mostly theoretical, coming from animal and lab studies suggesting ephedra might speed up a liver enzyme called CYP1A2 and clear out drugs that use it a bit faster. For your pain medicine, this enzyme isn't the main pathway, so any real change in how well it works is uncertain and probably small.

What I'd worry about more is the bigger picture: ephedra is a strong stimulant that raises heart rate and blood pressure, while your opioid can cause drowsiness and slowed breathing. Mixing those is stressful on the body. Ephedra is also banned in U.S. supplements for safety reasons. Please loop in your pharmacist or doctor before combining anything like this.

Here's the concern with pairing ephedra and Apadaz (benzhydrocodone with acetaminophen). The worry isn't the opioid part. It's the acetaminophen, which is hard on the liver if you take too much or stack it with other things that stress the liver. Ephedra-containing products have been tied to a number of liver injury cases on their own, so combining the two could, in theory, add up to more strain on your liver.

To be honest, this connection is theoretical and based on how these substances behave, not on solid studies of the two used together. Still, ephedra is banned in U.S. supplements for good reason, and I'd steer you away from it entirely. Please run any supplement past your pharmacist or doctor first, and watch your total daily acetaminophen.

What Is Benzhydrocodone and Acetaminophen Used For?

Brand name: Apadaz

Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.

Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
