Codeine, Paracetamol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ephedra interacts with.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

First things first: ephedra is banned in supplements in the U.S. because of serious heart and stroke risks, so I wouldn't reach for it at all. As for combining it with Co-codamol (codeine plus paracetamol/acetaminophen), the interaction we have on file is mostly theoretical. It comes from animal and lab work suggesting ephedra might speed up a liver enzyme called CYP1A2, which could clear certain drugs a little faster.

Honestly, codeine isn't mainly handled by that pathway (it's activated by a different enzyme, CYP2D6), and paracetamol clearance isn't strongly tied to it either, so any real-world effect here is uncertain and likely small. The bigger concern is ephedra's stimulant strain on your heart and blood pressure. Please check with your pharmacist or doctor before mixing these.

The piece of co-codamol I want to focus on here is the paracetamol (acetaminophen). At normal doses it's gentle on the liver, but in larger amounts or over time it can stress this organ. Ephedra-containing supplements have their own track record of liver injury, with a number of reported cases. So the concern is simple: two things that can each be hard on the liver, taken together, might add up.

I'll be honest, this is a theoretical worry based on how each works, not proven cases of the combo. Still, since ephedra is banned in U.S. supplements and carries real risks of its own, I wouldn't pair them. Watch for warning signs like yellowing skin or eyes, dark urine, or right-sided belly pain, and check with your pharmacist or doctor before using either one.
