Ephedra and Flutamide

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Flutamide

Flutamide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ephedra interacts with.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

First things first: ephedra is banned in U.S. supplements because it can dangerously raise blood pressure and stress the heart, so I wouldn't recommend it regardless. As for how it might mix with flutamide, the concern is that ephedra may speed up a liver enzyme called CYP1A2, which helps process flutamide. If that enzyme runs faster, your body could clear the medication a bit quicker, which in theory might make it slightly less effective at blocking androgens and controlling your prostate cancer.

I want to be honest about the evidence here: this is only based on lab and animal studies, so it's theoretical, not proven in people. Still, because this cancer treatment really matters, please don't add ephedra. Check with your oncologist or pharmacist about safe options.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Flutamide page

“A man taking flutamide (Eulexin) to block testosterone during prostate cancer treatment cannot afford big swings in the drug's level, and most of the 189 supplements on this page are capable of causing exactly that. The bulk of them work through the liver enzyme CYP3A4, which breaks flutamide down.”

Ephedra (ma huang) contains powerful stimulants like ephedrine that affect the heart and nervous system. About Flutamide

What Is Flutamide Used For?

Brand names: Eulexin, Euflex

Eulexin (flutamide) capsules are used to treat prostate cancer in men. They are always used together with an LHRH agonist (a hormone-lowering injection), never alone.

Eulexin is indicated for two stages of prostate cancer: locally confined Stage B2-C prostate cancer (used alongside goserelin acetate and radiation therapy, starting eight weeks before radiation begins) and Stage D2 metastatic prostate cancer (cancer that has spread beyond the prostate, taken with an LHRH agonist until the disease progresses).
