Olodaterol Hydrochloride, Tiotropium Bromide falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Here's the concern with Stiolto Respimat. One of its ingredients, olodaterol, is a long-acting beta agonist, and like other drugs in that family it can slightly stretch out the heart's electrical cycle (what we call the QT interval). On its own that's usually not a problem. Ephedra is a powerful stimulant that can do the same thing, plus it speeds up the heart and raises blood pressure. Stacking the two could add up.

Together, the worry is a higher chance of a dangerous irregular heartbeat, along with a racing heart, palpitations, jitteriness, or chest tightness. Ephedra is banned in U.S. supplements for good reason, so I wouldn't use it at all. Please talk with your pharmacist or doctor about safer choices before changing anything.

Your Stiolto Respimat inhaler contains olodaterol, which is a long-acting beta-agonist. It gently stimulates the heart and opens your airways. Ephedra is also a stimulant, so stacking the two could add up and push your heart harder than intended. In practice that might mean a faster or pounding heartbeat, jitteriness, tremor, trouble sleeping, or a rise in blood pressure.

To be fair, this concern is theoretical, based on how these compounds work rather than strong studies. Still, the bigger issue is ephedra itself, which is banned in U.S. supplements because of serious heart and stroke risks. I would steer clear of it entirely and ask your pharmacist or doctor about safer options before adding anything to your routine.
