Ondansetron falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ephedra interacts with.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

Here's the basic idea with these two together. Ondansetron (Zofran) is one of the medicines your liver clears partly through an enzyme called CYP1A2. Some animal research hints that ephedra may speed that enzyme up, which could move ondansetron out of your body a little faster. In theory, that might make it work less well, so the nausea and vomiting it's meant to control could break through.

I'll be honest with you though: this is weak, lab-and-animal evidence only, not something proven in people, so I wouldn't lose sleep over it. The bigger concern is ephedra itself. It's a strong stimulant that's banned in U.S. supplements because it can dangerously raise heart rate and blood pressure. Please skip it and ask your pharmacist about safer options.

About Ondansetron

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
