Tramadol, Acetaminophen falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ephedra interacts with.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

Here's the honest picture for your Tramacet (tramadol with acetaminophen). The concern on file is that ephedra might speed up an enzyme called CYP1A2, which could lower blood levels of drugs cleared by it. But I want to be upfront: this is based only on lab and animal research, so it's theoretical, and CYP1A2 isn't even the main pathway for either tramadol or acetaminophen.

The bigger, very real worry is different. Ephedra is a banned stimulant that raises heart rate and blood pressure, and tramadol carries its own risk of seizures and serotonin-related effects. Combining them is genuinely risky for your heart and nervous system. Please don't use ephedra, and talk with your pharmacist or doctor about safer options.

Here's the concern with combining ephedra and Tramacet (tramadol plus acetaminophen). Ephedra-containing products have been tied to a number of reports of liver injury, and acetaminophen is itself hard on the liver in higher amounts. Putting two things that can stress the liver together could, in theory, add up and increase the chance of harm.

I'll be honest: this particular warning is theoretical, based mostly on how these substances behave rather than strong studies. Still, ephedra is banned in U.S. supplements for good reason, and I wouldn't recommend it at all. Watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. Please talk with your pharmacist or doctor before mixing these, and ask about safer options.
