Theoretically, green coffee may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Green coffee can contain caffeine. Caffeine is reported to have antiplatelet activity. Theoretically, caffeine in green coffee might increase the risk of bleeding when used concomitantly with these agents. However, this interaction has not been reported in humans. There is some evidence that caffeinated coffee might increase the fibrinolytic activity in blood.

Tirofiban (Aggrastat) is a powerful IV antiplatelet drug given in the hospital, usually during heart procedures or for certain types of chest pain. It keeps your platelets from clumping together so a clot does not form. The concern here is simply additive: green coffee contains caffeine, and caffeine is thought to have a mild blood-thinning, antiplatelet effect of its own. Stacked on top of tirofiban, the theory is you could bleed a little more easily, showing up as bruising, gum bleeding, or oozing at IV sites.

That said, this is theoretical only and has not actually been reported in people. The blood-thinning effect of caffeine is small compared to a drug like tirofiban. Since tirofiban is given under close monitoring in a hospital, the team is already watching your bleeding risk. Still, it is worth mentioning any green coffee supplements to your nurse or pharmacist.

Tirofiban hydrochloride injection — available as Aggrastat and under the generic names Tirofiban and Tirofiban Hydrochloride — is used intravenously in people with non-ST elevation acute coronary syndrome (NSTE-ACS), a type of unstable heart condition where blood flow to the heart is dangerously reduced.

The goal is to lower the combined risk of death, heart attack (myocardial infarction), or persistent ischemia (insufficient blood flow to the heart) that may require a repeat cardiac procedure.
