Green Coffee and Diltiazem

Here is the interaction we found between these two, with the evidence behind it.

Minor

ANTIHYPERTENSIVE DRUGS

Why this applies to Diltiazem

Diltiazem falls under ANTIHYPERTENSIVE DRUGS — the group Green Coffee interacts with.

Theoretically, taking green coffee with antihypertensive drugs might increase the risk of hypotension.
Clinical studies have shown that green coffee extract decreases blood pressure. When used with antihypertensive drugs, green coffee might have additive blood pressure-lowering effects. However, this has not been shown in clinical research.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: Diltiazem already does the work of relaxing your blood vessels and easing your blood pressure down. Green coffee, thanks to its chlorogenic acid, may nudge blood pressure a little lower too. Put the two together and those effects could add up, so in theory your numbers might drop a bit more than expected.

The good news is this is rated a minor interaction, and the evidence is fairly weak, so for most folks it's not a big deal. Still, keep an eye out for signs your pressure is running low, like dizziness, lightheadedness when you stand up, or feeling unusually tired. Keep in mind green coffee also has caffeine, which can do the opposite and bump heart rate up. If you're thinking about a concentrated supplement, check with your pharmacist or doctor first.

From our pharmacist’s overview on the Diltiazem page

“Someone on diltiazem (Cardizem, Tiazac) is usually managing blood pressure, angina, or a fast heart rhythm, so this list deserves genuine care. Seven of the 260 ingredients on file carry a major rating, and two of those are things people actually buy: grapefruit, which blocks the enzyme that clears diltiazem and can push its blood leve…”

About Green Coffee

What is Green Coffee?

Green Coffee
  • Weight loss
  • High blood pressure
  • Blood sugar control
  • Antioxidant support
  • General energy

Green coffee is the unroasted bean of the coffee plant, prized for its high chlorogenic acid content. Some early studies suggest modest benefits for weight, blood pressure, and blood sugar, but the evidence is limited and far from conclusive. It contains caffeine, so people sensitive to stimulants should use caution and check with a pharmacist or doctor first.

About Diltiazem

What Is Diltiazem Used For?

Brand names: Cardizem, Cardizem SR, Diltiaz SR, Cardizem CD, Tiazac, Diltiaz XT, Diltiaz CD, Dilacor, Diltiaz, Cardizem LA, Dilacor XR, Cartia XT, Tiamate, Adizem-SR, Adizem-XL, Angitil SR, Angitil XL, Calcicard CR, Dilcardia SR, Dilzem SR, Dilzem XL, Slozem, Tildiem LA, Tildiem Retard, Viazem XL, Zemtard 120XL, Zemtard 180XL, Zemtard 240XL, Zemtard 300XL

Oral extended-release forms of diltiazem — including Matzim LA, Cardizem LA, Cartia, Cartia XT, Tiadylt ER, and various generic capsules and tablets — are used to treat high blood pressure (hypertension) and chronic stable angina (chest pain brought on by activity). Some capsule formulations, including Cartia, Cartia XT, and certain generics, are also approved for angina due to coronary artery spasm (sudden chest pain caused by a sudden tightening of a coronary artery).

Injectable forms of diltiazem are used in hospital settings to temporarily slow a dangerously fast heart rate in atrial fibrillation or atrial flutter (rapid, irregular heart rhythms), and to restore a normal heart rhythm in paroxysmal supraventricular tachycardia (PSVT) — sudden episodes of a very fast but regular heartbeat originating above the heart's lower chambers.
