Treprostinil falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Green Coffee interacts with.

Theoretically, green coffee may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Green coffee can contain caffeine. Caffeine is reported to have antiplatelet activity. Theoretically, caffeine in green coffee might increase the risk of bleeding when used concomitantly with these agents. However, this interaction has not been reported in humans. There is some evidence that caffeinated coffee might increase the fibrinolytic activity in blood.

Here's the thing to keep in mind about treprostinil (Orenitram): it's a medication used for pulmonary arterial hypertension, and one of its known properties is that it can make your platelets a little less sticky. In other words, it already has a mild blood-thinning effect of its own. Green coffee contains caffeine and chlorogenic acid, and in theory caffeine may also nudge platelets in that same direction.

So the concern is simply additive: stacking two things that both slightly discourage clotting could, in theory, raise your chance of bruising or bleeding more easily. I'll be honest though, this is theoretical and hasn't actually been reported in people. Modest coffee or food amounts are unlikely to matter much. If you're using concentrated green coffee supplements, just run it by your pharmacist or doctor first.

Theoretically, taking green coffee with antihypertensive drugs might increase the risk of hypotension.
Clinical studies have shown that green coffee extract decreases blood pressure. When used with antihypertensive drugs, green coffee might have additive blood pressure-lowering effects. However, this has not been shown in clinical research.

Treprostinil (Orenitram) is a medicine that widens blood vessels to ease the workload on your heart and lungs in pulmonary hypertension. One of its built-in effects is lowering blood pressure, and that's where green coffee comes in. Green coffee extract has been shown in some studies to nudge blood pressure down a bit on its own, so stacking the two could, in theory, drop your pressure a little more than expected.

If that happened, you might notice dizziness, lightheadedness, headache, or feeling flushed, especially when standing up. The good news is this is rated a minor concern, and it hasn't actually been proven in people taking them together. Everyday coffee amounts are unlikely to matter much; concentrated green coffee supplements are more likely to add up. Before starting one, just run it by your pharmacist or doctor.

About Treprostinil

What Is Treprostinil Used For?

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
