Here is the interaction we found between these two, with the evidence behind it.
PENTOBARBITAL (Nembutal)
Guarana contains caffeine. In vivo evidence suggests that caffeine can negate the hypnotic effects of pentobarbital in humans. However, animal research suggests that guarana does not alter the hypnotic effect of pentobarbital.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: pentobarbital is a barbiturate, a strong sedative meant to calm the brain and help you feel drowsy or sleepy. Guarana is loaded with caffeine, which does the opposite by blocking the sleep signal in your brain and waking you up. So when you pair them, the guarana may push against the sedative and weaken how well the pentobarbital works. In plain terms, you might not get as sleepy or settled as the medicine intends.
I'll be honest about the evidence here: this is mostly theoretical, and the research is mixed (some human data suggests caffeine fights the sedation, while animal studies didn't show a change). Still, since pentobarbital is a serious medication, it's worth a quick chat with your pharmacist or doctor before mixing in guarana or other caffeine-heavy products.

From our pharmacist’s overview on the Pentobarbitone page
“Pentobarbitone is a barbiturate, one of the older and heavier sedatives, and the gap between a dose that helps sleep and one that slows breathing too far is not a generous one. That is why I read this particular list with genuine concern.”

About Guarana
What is Guarana?
- Energy and alertness
- Reducing fatigue
- Weight loss support
- Athletic performance
- Mental focus
Guarana is an Amazonian seed that is naturally high in caffeine, which explains most of its stimulant and energy effects. While it may give a short-term boost in alertness and reduce fatigue, strong evidence for weight loss or major performance gains is limited, and it carries the same risks as too much caffeine. Talk to your pharmacist or doctor before using it, especially if you have heart problems, anxiety, or are pregnant.

Pentobarbitone Is Linked to Lower Levels of One Nutrient
Natural Medicines records Pentobarbitone as a medicine that can lower these. This is not an interaction with Guarana — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
