Here's the situation with guarana and your ertugliflozin (Steglatro). Guarana is basically a concentrated source of caffeine, and the concern here isn't about your liver breaking the drug down differently. Ertugliflozin works in the kidneys, flushing extra sugar out through your urine, so guarana doesn't change how much drug is in your system.

What we're really talking about is caffeine's own mild, unpredictable effect on blood sugar. Some reports say it nudges sugar up, others say down, and honestly the evidence is weak and conflicting. The practical worry would be slightly bumpier glucose readings. This is rated minor and unlikely, so an occasional energy drink probably won't derail you. Just keep an eye on your sugar levels and check with your pharmacist if you use guarana regularly.
