Here is the interaction we found between these two, with the evidence behind it.
ANTICOAGULANT/ANTIPLATELET DRUGS
Why this applies to Aspirin, Pentazocine
Aspirin, Pentazocine falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Guarana interacts with.
In vitro and animal research suggests that guarana extract can inhibit platelet aggregation. This effect may be due to the caffeine in guarana, which is also reported to have antiplatelet activity. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Talwin Compound combines aspirin (a blood thinner that keeps platelets from clumping) with pentazocine (a pain reliever). The piece that matters here is the aspirin. In lab and animal studies, guarana, mostly because of its caffeine, also seems to make platelets a little less sticky. Stacked on top of aspirin, the theory is that bleeding could become slightly more likely.
I want to be honest about the evidence: this has only been seen in test tubes and animals, never reported in real people, so I would not lose sleep over it. Still, watch for easy bruising, nosebleeds, or bleeding gums. Heavy guarana energy products also pile on caffeine, which can raise your heart rate. Check with your pharmacist before adding it.

From our pharmacist’s overview on the Aspirin, Pentazocine page
“Talwin Compound pairs pentazocine, an opioid painkiller, with aspirin, and I read its supplement list with more concern than most. The great majority of entries do one of two things: raise the bleeding risk that aspirin already carries, or deepen the sedation pentazocine causes.”

About Guarana
What is Guarana?
- Energy and alertness
- Reducing fatigue
- Weight loss support
- Athletic performance
- Mental focus
Guarana is an Amazonian seed that is naturally high in caffeine, which explains most of its stimulant and energy effects. While it may give a short-term boost in alertness and reduce fatigue, strong evidence for weight loss or major performance gains is limited, and it carries the same risks as too much caffeine. Talk to your pharmacist or doctor before using it, especially if you have heart problems, anxiety, or are pregnant.
