Here's the honest picture with guarana and your Toujeo (insulin glargine). Guarana is basically a big dose of caffeine, and caffeine has a bit of a reputation for nudging blood sugar in either direction. The reports are mixed, and the effect is usually small, so this is considered a minor, unlikely interaction rather than a dangerous one.

Where I pay a little more attention is that caffeine can mask the early warning signs of low blood sugar, like a racing heart or the shakes, since caffeine itself causes those feelings. If you add guarana on top of your insulin, you might not notice a low as quickly. Keep an eye on your readings if you start or change a guarana product, and check in with your pharmacist or doctor before making it a regular habit.
