Grepafloxacin falls under QUINOLONE ANTIBIOTICS — the group Guarana interacts with.

Theoretically, quinolone antibiotics might increase the levels and adverse effects of caffeine.
Guarana contains caffeine. Theoretically, concomitant use might increase serum caffeine concentrations and the risk of adverse effects. Quinolones (also called fluoroquinolones) can decrease caffeine clearance by inhibiting the cytochrome P450 1A2 enzyme.

Here's the simple version: guarana is basically a concentrated source of caffeine. Grepafloxacin, like other fluoroquinolone antibiotics, can slow down the liver enzyme (CYP1A2) that normally clears caffeine out of your body. So while you're on this antibiotic, that caffeine sticks around longer and can pile up. Just to be clear, it's the caffeine building up here, not the antibiotic, so this won't change how well the infection is treated.

What that feels like is the usual too-much-caffeine stuff: jitteriness, a racing or pounding heart, trouble sleeping, and feeling anxious or wired. The evidence for this is fairly modest and the effect is usually minor, but if you're sensitive to caffeine or already get the jitters, go easy on guarana and check with your pharmacist or doctor.
