Etonogestrel falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Guarana interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Guarana contains caffeine. Caffeine is metabolized by the CYP1A2 enzyme. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.

Here's a twist worth understanding: this interaction isn't really about your etonogestrel (Implanon) implant changing. It runs the other direction. Etonogestrel can mildly block CYP1A2, the same liver enzyme that breaks down the caffeine in guarana. So if that enzyme is slowed down, caffeine may hang around in your body a bit longer than usual.

In plain terms, the guarana may hit you harder. You might notice more of the usual caffeine effects, like jitteriness, a racing heart, trouble sleeping, or feeling on edge. This is only a theoretical, lab-based concern rated as minor, so it's nothing to panic over. Your birth control should still work fine. If you're sensitive to caffeine, just go easy on guarana and check with your pharmacist or doctor.

Here's the thing to know about pairing guarana with your Implanon (etonogestrel) implant: this interaction is really about the caffeine in guarana, not your birth control. Hormonal contraceptives can slow down how fast your body clears caffeine, by roughly 40% to 65%. So the caffeine from guarana may stick around longer and hit a little harder than you'd expect.

In plain terms, you might feel more of the usual caffeine effects: a racing or pounding heart, jitteriness, trouble sleeping, or feeling anxious. Your implant keeps working just fine, and this is rated minor and partly theoretical, so don't panic. Just go easy on guarana and other caffeine sources, and check with your pharmacist if you feel wired or uncomfortable.
