Acetaminophen, Caffeine, Codeine, Salicylamide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

Here's the good news: this is a very minor, mostly theoretical concern. The piece that overlaps is the caffeine in your combination product. In lab dishes, red clover can slow down CYP1A2, the enzyme that clears caffeine. If that happened in real life, caffeine could linger a bit longer and you might feel jittery, have trouble sleeping, or notice a faster heartbeat.

But a small human study actually checked this. Postmenopausal women took standardized red clover isoflavones twice daily for two weeks, and it did not change how their bodies handled caffeine. So in practice, this interaction is unlikely to amount to much. The codeine and acetaminophen parts aren't the issue here. Still, if you're sensitive to caffeine or have questions, a quick chat with your pharmacist is always worthwhile.

Here's the gist for this combination product (which pairs acetaminophen, caffeine, codeine, and salicylamide). The concern on file is that red clover's isoflavones might block an enzyme called CYP3A4 in the lab. If that happened in your body, drugs cleared by that pathway could build up a little. In theory that could mean slightly stronger codeine-type effects, like extra drowsiness or constipation.

But here's the reassuring part: this is mostly a test-tube finding, and an actual human study using a standardized red clover dose showed no meaningful change in a CYP3A4 drug. So a real-world problem is unlikely and would be minor at most. Still, since codeine is involved, it's worth a quick word with your pharmacist or doctor before combining them.

Theoretically, soy might reduce the clearance of caffeine; however, a small clinical study found no effect.
Red clover contains genistein. Taking genistein 1 gram daily for 14 days seems to inhibit caffeine clearance and metabolism in healthy females. However, this effect does not seem to occur with the lower amounts of genistein found in red clover. A clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine.

The piece of this combo pill that red clover might touch is the caffeine. Red clover's isoflavone, genistein, can in theory slow down how your body clears caffeine. If that happened, caffeine could linger a bit longer and you might feel jittery, have a racing heartbeat, or struggle to sleep.

Here's the reassuring part though: this slowdown was only seen with very high, concentrated genistein doses (about 1 gram daily). At the much smaller amounts found in normal red clover supplements, a study in postmenopausal women showed no real effect on caffeine at all. So for most people this is a minor, mostly theoretical concern. If you're sensitive to caffeine, just mention it to your pharmacist or doctor.
