Yerba mate contains caffeine. Caffeine is metabolized by CYP1A2. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.

Here's the simple version: this interaction is really about the caffeine in your yerba mate, not your HIV medicine itself. Your body uses an enzyme called CYP1A2 to clear caffeine out of your system. Atazanavir and cobicistat can slow that enzyme down a bit, so the caffeine from mate may hang around longer than usual.

In real life, that could mean the mate hits you harder: more jitteriness, a racing or pounding heart, trouble sleeping, or feeling wired. The good news is this is rated minor and is only theoretical, based on how the drugs work rather than reported cases. A cup or two is usually fine. If you drink mate often and notice you feel extra buzzed, ease back and mention it to your pharmacist or doctor.

Here's the gist with your Evotaz (atazanavir plus cobicistat) and yerba mate. Your atazanavir is partly cleared by a liver enzyme called CYP3A4. Lab studies hint that yerba mate may slow that same enzyme down a little. In theory, that could let atazanavir levels drift a bit higher, which might mean more of its usual effects like nausea, headache, or the yellowing of skin or eyes (hyperbilirubinemia) that this drug is known for.

I want to be honest, though: this is rated minor and rests only on test-tube data, not real people. A cup or two of mate likely won't move the needle much. Since HIV therapy needs steady, reliable levels, just mention your mate habit to your pharmacist or doctor before changing anything.

What Is Atazanavir and Cobicistat Used For?

Brand name: Evotaz

Evotaz (atazanavir/cobicistat) is used to treat HIV-1 infection. It must always be taken together with other antiretroviral medicines — it is not a complete regimen on its own. It is approved for adults and for pediatric patients who weigh at least 35 kg.

In patients who have been on HIV treatment before (treatment-experienced), how well Evotaz works depends on how much resistance to protease inhibitors the virus has already developed. Your prescriber will review this before starting you on Evotaz.
